Health Tourism: Finding Patients from Abroad, a Digital Marketing Guide

A patient from abroad does not come on the day they see your ad, they come on the day they trust you. This guide covers every step of international patient acquisition, from your clinic choosing a target country to the patient coordinator's first message, together with platform policies and the 2025 regulation.

Many clinics start the international patient business with ads and end up disappointed by ads. Clicks come in, forms come in, WhatsApp messages come in, but appointments do not. The problem is usually not the ad but what happens before and after it. The market is large and still growing. According to data from the Service Exporters' Association, Turkey welcomed more than 1,398,000 people who came to receive health care in 2025. 1. Choosing a Target Country: Market First, Budget Second

"Let's advertise in Europe" is not a strategy. A dental patient in Germany and a hair transplant patient in the Gulf search differently, compare differently and decide differently. Answer four questions when choosing a target country:

  • Is there demand? Search for the treatment name in the target language and target country in Google Ads Keyword Planner. The volume and competition for searches such as "Hair transplant Turkey" and "Zahnimplantat Türkei" give you the first signal.
  • Why would they come to you? Waiting times in the patient's country, the cost difference, flight time and ease of getting a visa are the real drivers of the decision. These reasons vary from market to market.
  • Can you speak that language? If you run the ad in German and answer the phone in English, you lose the patient in the first minute. The languages your coordinator team speaks limit your list of target countries.
  • What are competitors doing? Search for clinics selling the same treatment as you in the target country and review their ads in the Google Ads Transparency Center and the Meta Ad Library.

Choose two, or at most three, markets at the start. A budget spread across five countries at once does not generate enough data to learn in any of them. Expanding once the winning market is clear is far more efficient than trying to be visible everywhere from day one.

2. A Multilingual Website: Localization, Not Translation

If the language the patient searches in is not the language your website speaks, the ad money melts away on the landing page. Three rules are decisive when building a multilingual site.

German content should live on separate pages under /de/ and Arabic content under /ar/, with the pages linked to each other through hreflang tags. Setups that switch languages with a dropdown on a single page appear as a single language in search engines. For right-to-left languages such as Arabic, the page layout also needs to be mirrored.

Machine translation breaks trust

A patient making a health decision notices the slightest oddity in the language. Treatment pages, process descriptions instead of price information and FAQ sections should be written, or at least reviewed, by a native speaker of that language. A German-speaking patient expects formal and detailed information, while a patient in the Gulf places more weight on family and privacy. Translating the same text is not enough, the message has to be built for each market.

A separate landing page for each treatment

Hair transplants, implants and rhinoplasty are not explained on the same page. For each treatment and each language, build a page that clearly shows the process, length of stay, the doctor and the contact route on a single screen. On mobile, the WhatsApp button should always be visible.

3. Google and Meta Health Advertising Policies

Health advertising is one of the areas platforms police most strictly. A campaign set up without knowing the rules is either not approved, or approved and then quietly restricted.

Google Ads

  • The healthcare and medicines policy varies by country. Some areas, such as prescription drugs, online pharmacies and addiction treatment, require certification, and ads for unproven or experimental treatments are restricted. Check the rules for each target country separately in the Google Advertising Policies Help Center.
  • The personalized advertising limit. Google's personalized advertising policy treats health as a sensitive interest category. Businesses advertising in this category cannot use advertiser-created audiences such as customer match and remarketing lists. Google's own predefined audiences can still be used. So the "show it again to everyone who visited the site" setup does not work in health, and search intent and content quality matter even more.
  • Promise language. Phrases such as "guaranteed results", "painless" and "permanent solution" are risky from both a policy and a legal standpoint.

Meta (Instagram and Facebook)

  • 18+ age limit. Meta requires ads for cosmetic procedures and surgery to be targeted at people aged 18 and over.
  • Before and after images. For cosmetic procedures and surgery, including hair transplants, images showing a before and after transformation are allowed with targeting set to 18 and over. For weight loss products, such comparisons are restricted. Copy and questions that negatively target a person's appearance, a body part or their hygiene are rejected. That is why a headline such as "Are you embarrassed by your thinning hair?" will not be approved.
  • Data restrictions. From January 2025, Meta began restricting the sharing of lower-funnel conversion events from data sources it classifies as health and wellness. Check in Events Manager, under the data source category, whether the restriction applies in your market, and set up optimization accordingly.

4. Turkey's Health Tourism Regulation: Do Not Start Without an Authorization Certificate

The Regulation on International Health Tourism and Tourist Health was published in the Official Gazette on April 26, 2025 and entered into force. Before allocating budget to marketing, confirm your clinic's compliance with this regulation, because promoting a non-compliant facility abroad carries legal risk on top of reputational risk.

  • Authorization certificate: Under the regulation, the authorization certificate for a health facility is issued by the Ministry of Health. Authorization procedures for intermediary agencies are handled by USHAŞ (International Health Services Inc.). If you work with an intermediary that refers patients to you, ask to see its certificate.
  • HealthTürkiye integration: Facilities and intermediary agencies are integrated with the HealthTürkiye portal the Ministry set up for registration, tracking and inspection.
  • Foreign language website: The regulation's promotion and information article makes a website with a foreign language option mandatory for health facilities and intermediary agencies. In other words, a multilingual site is no longer just a marketing choice, it is a legal requirement.
  • Language obligation: Intermediary agencies are required to provide call center service in at least two foreign languages on a 7 days, 24 hours basis. This means the language support your marketing promises must also be backed by your operations.
  • Insurance and accreditation: The regulation introduced complication insurance and accreditation requirements for surgical procedures and granted existing facilities transition periods to comply. Verify where your clinic stands on this timeline through Ministry announcements and your legal advisor.

On promotion, the regulation refers to the existing legislation that health facilities are subject to. In Turkey, the promotion of health institutions is limited to informational purposes, and exaggerated promises, guarantee statements and misleading comparisons are prohibited. Check your ad copy against both the platform policy and these rules together.

Patient reviews and images must comply with the law

A before and after image, a video testimonial or a treatment story reveals the patient's health data, and under KVKK (Turkey's personal data protection law) health data counts as special category personal data. If you are going to use this content, obtain the patient's explicit consent and document in writing which channels and what period the consent covers. The data protection rules of the patient's own country may also apply, so take GDPR into account for patients from Europe. A single image used without consent can undo years of reputation with one complaint.

6. WhatsApp and the Patient Coordinator: This Is Where the Appointment Is Won

International patients usually make first contact by message rather than a form. That is why the patient coordinator is a marketing channel as important as the ads. A well-run coordinator setup has four features:

  1. Speed: Patients write to several clinics at once. A personal reply that arrives in the first few minutes and addresses them by name and in their language is worth more than a long text the next day. Plan shifts with time zone differences in mind.
  2. Native language: Reply in Arabic to a patient who writes in Arabic and in German to one who writes in German. A conversation carried on through automatic translation breaks trust in a health matter.
  3. A structured pre-consultation: The coordinator collects photos, medical history and expectations through a standard flow and prepares a file ready for the doctor's assessment. Since you are collecting health data, make sure your messaging channel and storage method comply with data protection rules.
